At a Glance

• Basic Life and AD&D at no cost: 1.5 times your annual earnings, up to $350,000. Coverage over $50,000 is taxable income.
• Voluntary Life/AD&D: buy coverage for yourself in $10,000 steps (guaranteed up to $300,000), your spouse in $5,000 steps (up to $25,000), and children ($10,000).
• Short-Term Disability (company paid): 60% of pay up to $1,500 a week for up to 12 weeks after a 7-day wait. You can buy up to $2,000 a week.
• Long-Term Disability (company paid): 60% of pay after 90 days of disability. You can buy up to $12,500 a month.
• Review or name your beneficiaries in Ceridian during enrollment.
