- High PPO: $500 / $1,000 in-network deductible, $15 office visit, $30 specialist. $260.03 semi-monthly for employee only.
- Standard PPO: $2,000 / $4,000 deductible, $20 office visit, $40 specialist. $84.97 semi-monthly for employee only.
- $3,400 HDHP (was $3,300, updated for IRS rules): $40.77 semi-monthly for employee only. HSA eligible.
- $5,000 HDHP: $20.38 semi-monthly for employee only. HSA eligible.
- Retail prescriptions (in-network, 30 days): Tier 1 $10; Tier 2 $25–$30; Tier 3 $45–$60; Tier 4 20% up to $250.
- Medical premiums go up in 2026. The 2025 Wellworks Wellness Deal credit is not offered in 2026.
